This form allows the person who is giving a presentation to give permission to a producer (can be an individual or company) to film that presentation for a specific purpose (e.g. training video). This form includes provisions which prohibit the producer from using the film in any other way or selling it.

This form can be used in the following provinces: Alberta, British Columbia, Manitoba, New Brunswick, Newfoundland and Labrador, Northwest Territories, Nova Scotia, Nunavut, Ontario, Prince Edward Island, Saskatchewan and Yukon.
